Screening delays increase the risk of advanced cancer and metastasis. This study assessed the prevalence of non-use of cervical cancer screening in Khouribga and identified associated risk factors. A cross-sectional study was done with 499 women aged 30 to 49 years who visited the Khouribga healthcare centers between March and December 2024. A face-to-face questionnaire was used to collect information about sociodemographic variables, knowledge of cervical cancer, screening awareness, healthcare interactions, and social influences. A multivariate analysis found key risk factors for not using cervical cancer screening. Among the 458 participants (91.8%) who had not undergone screening, the mean age was 38.9 ± 6.1 years. The main risk factors for not participating in cervical cancer screening were lack of awareness about the target screening population (APR = 10.83; CI [2.22–52.7]) and unawareness of the availability of free screening services (APR = 9.82; CI [2.69–35.7]) . The high prevalence of non-use of cervical cancer screenings highlights the pressing need for focused awareness initiatives. To increase participation and lessen the burden of cervical cancer, education must be strengthened, and the availability and eligibility of free screening services must be made obvious.  \nKeywords non-use of screening, cervical cancer, lack of awareness, free testing, target population  \nCervical cancer is a public health issue and the fourth most frequent disease in women, with an estimated 660,000 new cases and 350,000 deaths globally in 20221. Every two minutes, a woman dies from cervical cancer2. Over 94% of these deaths happen in low-and middle-income countries1. These disparities are associated with social determinants of health and unequal access to screening and treatment services1.  \nIn Morocco, cervical cancer is the second most frequent cancer in women, following breast cancer3. The Cancer Registry of the Greater Casablanca Region has a standardized incidence of 14.1 per 100,000 women. The projected number of new cases in 2020 was around 33804.  \nCervical cancer is caused by the human papillomavirus (HPV), which is primarily transferred sexually5. Combined prevention is the most effective technique for lowering cervical cancer incidence and fatality rates. This method is a multifaceted strategy that includes primary, secondary, and tertiary prevention measures such as HPV vaccination to prevent initial infection, safer sexual behaviors, screening for precancerous cervical lesions, and treating these lesions and existing cancers. This preventive method identifies precancerous lesions and prevents cervical cancer.
