As coinvestigators in research disciplines grounded in rigorous methodology and evidence-based practice, we must confront not only what we discover, but how we communicate and disseminate those discoveries.  \nThe recent consensus recommendations published in the British Journal of Anaesthesia1 in February 2026 on the responsible use of LLMs in manuscript preparation and peer review, to which I contributed as a coinvestigator, represent a timely and necessary milestone in medical publishing.  \nJust like in anesthesiology, clinical practice, education, and multidisciplinary research are essential for a well-rounded respiratory care practice. Since evidence-based medicine is the foundation of respiratory care practice, our community thriveson clarity of evidence, methodological integrity, and transparent reporting.2 There are several applications of artificial intelligence (AI) in respiratory care clinical practice.3 Now, as respiratory care researchers and clinicians increasingly engage with digital tools to streamline literature reviews, draft manuscripts, andrefine texts, it is essential to set discipline-specific standardson when, how, to what extent, and whether such technologies should be used.  \nThe Delphi consensus by De Cassai et al. underscores several principles about the use of LLMs that must be adopted by the Indian Journal of Respiratory Care (IJRC) .1 The consensus emphasizes that, to maintain the highest standards of academic integrity, authors must prioritize transparency and intellectual ownership when utilizing LLMs. While these tools can assist in the writing process, authors are strictly prohibited from publishing plagiarized material and must explicitly disclose the extent of AI involvement to ensure readers and reviewers maintain a clear perspective on the work’s origins. Ultimately, the crux of the research, specifically the interpretation of data and clinical insights, must be the original product of human thought. This ethical boundary is nonnegotiable in specialized fields like respiratory care, where the nuances of clinical recommendations require human accountability to prevent bias and ensure patient safety.  \nImplementing these recommendations within the IJRC represents a proactive step toward safeguarding the journal’s legacy of excellence.
