Primary (congenital or idiopathic) disease may result from immune suppression of marrow by autoreactive T lymphocytes, toxic injury to stem and/or progenitor cells from agents such as chemotherapy or drugs, or inherited intrinsic stem cell defects such as Fanconi anemia. Typical clinical features include anemia with fatigue, dizziness, bruising and gum bleeding; thrombocytopenia with epistaxis, menorrhagia, retinal hemorrhage, petechiae and purpura; and neutropenia causing prolonged infections and spontaneous mucosal haemorrhages, with no organomegaly or lymphadenopathy. Diagnosis relies on CBC, blood film, and bone marrow examination showing marked marrow hypocellularity with confirmation by cytogenetic testing to exclude inherited forms and myelodysplastic syndromes.
