The pathophysiology includes impaired kidney filtration and retention of nitrogenous wastes such as urea and ammonia, alongside dysregulation of extracellular volume and electrolytes. Causes are organized into pre-renal, renal, and post-renal categories: decreased perfusion (e.g., cardiac failure, sepsis, blood loss, dehydration, vascular occlusion), intrinsic kidney injury dominated by acute tubular necrosis (hypoxic, toxic, or septic), and obstruction of urine flow (calculi, fibrosis, prostatic disease, malignancy, strictures).
