In Table 1, the authors review the cases of 10 patients with acute intraparenchymal hemorrhage (IPH) because of MMA pseudoaneurysm. I also experienced a similar case.2) In the most recent case report, computed tomographic (CT) angiography was useful for the diagnosis of pseudoaneurysm. However, in Fig. 1B, I could not find a definite pseudoaneurysm, as this figure shows the source image without reconstructive CT angiography. I recommend presenting CT angiography to show MMA pseudoaneurysms. Such images can provide critical information for clinicians. \nIn most cases of acute traumatic pseudoaneurysm in the MMA, epidural hematoma (EDH) is the dominant presentation. Acute IPH because of pseudoaneurysm in the MMA is very rare. Moon et al.2) reported that traumatic pseudoaneurysm in the MMA resulted in EDH and IPH connected via a dural tear. Moreover, the pseudoaneurysm was located on the outer surface of the dura mater. Rupture of the MMA pseudoaneurysm resulted in EDH, and additional bleeding resulted in IPH via the dural tear, although cerebral contusion was not found in the operative field. \nIn the case reported by Mohammed et al.,1) I wondered about the exact location of the pseudoaneurysm in the dura mater (e.g., outer or inner surface) . I could not find EDH in Fig. 1A; therefore, it is possible that another mechanism resulted in IPH in this case. One possibility is the separate development of the pseudoaneurysm and the IPH, which might have resulted from cerebral contusion after the trauma. To identify the exact mechanism ofIPH, I recommend a more detailed description. \nLim et al.3) reported a case similar to that reported by Mohammed et al.,1) in which the patient’s skull appeared unremarkable. On the inner surface of the intact dura, \nCopyright © 2025 by KSCVS and KoNES [www.the-jcen.org](www.the-jcen.org) 385 \nPseudoaneurysm of the middle meningeal artery \nthere was an irregularly shaped aneurysm arising from the MMA and active bleeding from the injured right MMA. In this case, the pseudoaneurysm located on the inner surface of the dura mater might lead to the development ofIPH. \nMohammed et al. 1) reported that histopathology of the pseudoaneurysm revealed the presence of an aneurysmal sac with a tear lined by a fibrin clot. Pseudoaneurysms typically lack a well-defined aneurysmal sac.
