First, presenters introduce a Dementia Consortium of community, health, and public health representatives and people with lived experience that supports dementia-friendly training and education across primary care, long-term care, and community settings. This is followed by results from implementation of an innovative primary care model of dementia navigator services designed to support comprehensive and coordinated care for persons with dementia and to strengthen community-clinical linkages. Finally, presenters explore examples and outcomes of interventions designed to promote brain health, early detection, and diagnosis as well as caregiver health. By applying the HBI public health framework, communities can enhance local collaboration and coordination, raise public and provider awareness about dementia, and design cross-sector strategies to address the healthcare costs and challenges of care for people impacted by ADRD.","cbCaidhSR5tFRtSM","https://ap.wps.com/l/cbCaidhSR5tFRtSM","pdf",368558,"English","# Session 2065 (Symposium)\n## Partnerships and stakeholder engagement guided by the HBI Road Map\n## Dementia Consortium for training and education\n## Dementia navigator primary care model and care coordination\n## Interventions for brain health, early detection, diagnosis, and caregiver health","[{\"question\":\"What problem does the symposium address in Texas?\",\"answer\":\"It addresses the growing need for dementia care as Texas has an increasing number of older adults living with dementia.\"},{\"question\":\"How does the initiative guide the collaboration described in the session?\",\"answer\":\"It is guided by the CDC Healthy Brain Initiative (HBI) Road Map to engage stakeholders, identify needs, and strengthen the state’s dementia care infrastructure.\"},{\"question\":\"What solutions are presented to improve outcomes for people with ADRD and caregivers?\",\"answer\":\"The session covers a dementia consortium for dementia-friendly training, a dementia navigator primary care model for coordinated care, and interventions that promote brain health, early detection and diagnosis, and caregiver health.\"}]","A ENGAGEMENT STRATEGY TO BUILD A DEMENTIA CARE INFRASTRUCTURE - Symposium | PDF",1790625901]
