Log in [to](to UMR.com)[ ](to UMR.com)[UMR.com](to UMR.com) (if needed, see UMR Login for step-by-step instructions).  \n2. In the blue menu at the top, click on Health Center, select Live Well Reward$ under Wellness Programs.  \n3. Under Live Well Reward$, click Get Started.  \n4. Find the Clinical Health Risk Assessment tile in the middle column and click Get Started.  \n5. Complete the assessment by answering the questions as prompted. Quest lab results will auto-populate in the CHRA (it can take up to 90 days) or you can manually enter the data. If you have not done lab work yet, select “Don’t Know” for the lab values and Quest results will automatically populate later. For non-Quest results, you cannot go back later to enter the data.  \n6. Report:  \na. Once your lab values are entered, you will be able to download a personalized analysis report from UMR.  \nb. The message You have submitted your assessment will be replaced with You have completed your assessment, you will see a circle with your score, and the Review button will be replaced with View Report.  \nc.
