We strongly recommend using the Direct-to-Quest option or an onsite biometric screening instead. If you choose to use the Physician Lab Form, please upload it to Live Well Reward$, rather than faxing it, and upload it as early as possible to allow for the longer processing time.  \nUse the Physician Lab Form if you prefer to:  \n• Get lab work that’s ordered by your doctor which may include additional tests not required for a biometric screening (such as thyroid) .  \n• Use an in-network lab other than Quest (this requires your doctor to order the labs) .  \n• Go to a convenience care clinic such as CVS or Walgreens.  \nBiometric Screening (Physician Lab Form via UMR)   \n1. In the blue menu at the top, click on Health Center, then Live Well Reward$ under Wellness Programs.  \n3. Under Live Well Reward$, click Get Started.  \n4. In the Live Well Reward$ Wellness Platform, in the middle column, scroll down to the Physician Lab Form section.  \n5. Click the Download button to download and print your personalized form. See a sample form at right. Review the downloaded form to verify that your personalized information is accurate.  \nNOTE: The A1c test, which measures your average blood sugar level, is optional but available to all employees, with or without a diabetes diagnosis.  \n6. Complete your screening and upload your form prior to the November 30 deadline using one of the options below.  \na. Option 1-In-network lab such as BayCare or Labcorp:  \n• Have your doctor order the lab work. Make sure it includes all required items (A1c is optional) .  \n• Schedule an appointment at the lab of your choice. Do not use the Direct-to-Quest Patient Service Center appointment scheduler if using this method. It will generate an additional lab order that may cause confusion and problems later.  \n• When you receive the results, ask your doctor to complete and sign the form.  \nb. Option 2-Convenience care clinic such as Walgreens or CVS:  \n• Ask the pharmacy staff who does the screening to complete and sign the form.  \nSAMPLE  \n7. Upload your form:  \na. Sign and date the completed form.  \nb. Upload the form electronically on Live Well Reward$ in the Physician Lab Form section.  \n• Click Upload.  \n• Click Choose file and navigate to the file which can be a PDF if you scan it or an image file if you take a photo of it (jpg or png) . Click Upload.  \nc. Processing time is about 1 week for an uploaded form and 3 weeks for a faxed form due to manual processing. It is highly recommended that you upload the form instead of faxing.  \n8. Confirmation (Live Well Reward$ Platform) :  \na. Message: Click the envelope with a red dot at the top right of the screen to access a confirmation message that your Physician Lab Form has been received. The message will alert you if any lab results are missing or illegible.  \nIf the message says Missing Biometrics: None, your form is approved.  \nb. Checkmark: In the Track Your CHRA and Biometric Screening Progress section, click More to see a green checkmark next to Biometric Screening Completion status.  \nc.
